Treatment

• The Memory Disorders Clinic is available for self referral and by physician referral.
• Following the evaluation process, a comprehensive report including diagnostic impressions and treatment recommendations are forwarded to the referring provider.
• An individually tailored packet of diagnostic and supportive information is provided to the patient and family.

• Evaluation, management and treatment of patients with memory difficulties, cognitive related problems, and memory disorders including Alzheimer’s disease (AD), dementia with Lewy bodies (DLB), frontotemporal dementia (FTD), mild cognitive impairment (MCI) and vascular dementia (VaD)
• Neuropsychological, geriatric and psychosocial assessments

• Supportive education and resources for patients and care partners
